Virtual/Remote sessions start March 2023!

You will find all directives and information on this page regarding how to set up your appointment.

Exclusively to Florida residents, all counseling sessions are remote/virtual via a secure HIPAA compliant service.

Counseling sessions are available for clients who are ideal for x1 to x2 a month contact. If you need more than once or twice month session contact, it may be best to contact another provider.

Seasonal Short Term/Session In-Person Group Therapy may be offered x1 or x2 a year at a site in the state of Florida.

INSURANCE

Accepted In-Network Insurance

(Some plans are accepted. Please check to see if I am in network with your plan.)

Aetna

– Only Commercial plans with IDs starting with “W” (as of 2/1/24)

BCBS

– Florida Blue & Other Blue Cross Blue Shield Plans

Tri-Care/Humana Military Select PPO – (Non-Network Authorized Provider)

Chat N Relax Counseling & Consultation, LLC – Florida is currently an OUT-OF-NETWORK provider with all other Florida & Southeast Region insurance plans.

Chat N Relax Counseling & Consultation, LLC – Florida is currently accepting self-pay, Aetna, Florida BCBS, & Tri-Care clients only while pending contract with two other Florida/Southeast Region insurance plans.

APPOINTMENT DIRECTIVES

  • Call (201)675-1083 to have a free 15-minute consultation & inquire about availability.
  • Once availability is confirmed, DaRon Stephens, LMHC, MCAP will ask that you complete the forms below and email to chatrelaxcounseling@gmail.com.
  • Once forms are emailed back & reviewed, DaRon Stephens, LMHC, MCAP needs to approve completion. Your pending session will now be confirmed.
  • Please note: Do not shred or throw away your completed paperwork until DaRon Stephens, LMHC confirms that all paperwork is received and approved. DaRon Stephens, LMHC will inform you when it is ok to dispose of your paperwork.
  • Upon completion and approval of your forms, DaRon Stephens, LMHC, MCAP will send you an invitation to the secure HIPAA compliant service via the email you provide to officially schedule your session in the system.

PAYMENT INFORMATON

All payments will be made via credit/debit card only.

There is no $1.50 convenient/transaction fee for card payments due to services being 100% remote/virtual. (FL only)

Please be ready to provide your card information either over the phone or during your first session over the secure HIPAA compliant service.

No card payment information should be emailed or included on your forms!
