Services offered at Chat N Relax Counseling & Consultation, LLC
Individual Counseling & Therapy
Individual services are offered to clients who are seeking to work on their issues on a one on one level. There are no set limits on how many individual counseling sessions you may have. Client’s generally use individual counseling to work on personal issues that may not be appropriate in a group or family therapy. Individual counseling sessions can also be used for consultation involving mental health, behavioral, substance use, & general social services concerns.
Couples Counseling
This type of service is provided to a couple or two individuals who are seeking help with their relationship. Couples counseling explores communication, trust, and any general relationship concern that may be troubling the relationship.
Family Counseling
This type of service is offered to a client who would benefit from the involvement of other family members being part of counseling services. This type of counseling explores family relationships, functioning, and communication concerns. Family counseling is restricted to up to three (3) additional family members in session.
Substance Use Evaluation
This type of service is offered to clients who are referred by probation, legal, a state agency or entity, primary care provider, and any other official service providing entity requesting a substance use evaluation. A client and/or guardian may request this type of service however, the service may not be offered without proof of need. A substance use evaluation is completed to assess future services or level of care that a client may need to address substance use issues.
In order to be referred for this type of service, a client must provide documentation in writing from said service provider, legal, or state entity on company’s letter head requesting (referral) for substance use evaluation with explanation why the evaluation is needed. The requested documentation may be brought in by client or guardian AFTER referred provider was contacted by Chat N Relax Counseling & Consultation, LLC to verify the need of this type of service. Releases will need to be signed prior to phone contact to referred provider by Chat N Relax Counseling & Consultation, LLC. 30 Minutes Phone Service (For Active Ongoing Treatment Clients/Consumers Only)
Chat N Relax Counseling & Consultation, LLC understands that life stuff happens a lot in-between sessions. In the event you and/or your minor is unable to make an immediate appointment with DaRon Stephens, LPC, LCADC, LMHC, MCAP, LMT or simply cannot wait until your next scheduled appointment, the Thirty (30) Minute Phone Service may be an appropriate option for you.
This service allows you to have up to two (2) thirty (30) minute verbal conversation with DaRon Stephens, LPC, LCADC, LMHC, MCAP, LMT a month via phone to discuss your immediate concern or issue. This service is also available for case collaboration/management or general advisement. Please be advised that DaRon Stephens, LPC, LCADC, LMHC, MCAP, LMT will have this conversation securely, privately, and confidentially with you. It is your responsibility to be aware of your personal environment/ surroundings for confidentiality reasons/concerns if you chose to use this phone service.
If you attend counseling once (x1) a month, you are allowed to use this service once (x1) a month in addition to your regular once (x1) a month visit. This service is not meant to replace individual and/or family, group, couples counseling.
You must schedule your Thirty (30) Minute Phone Service at least 24 hours prior to proposed call. DaRon Stephens, LPC, LCADC, LMHC, MCAP, LMT will place the initial call on scheduled date & time to you and/or your minor. Please contact DaRon Stephens, LPC, LCADC, LMHC, MCAP, LMT to schedule the Thirty (30) Minute Phone Service call.
This option is not meant to be used for a crisis/emergency type of situation. In the event of an emergency, please contact local emergency authorities and/or 911. As always, you may call as usual for general inquires, to reschedule or book your next upcoming appointment however, if advisement and/or treatment recommendations are needed outside of the traditional office session setting, the fee will apply to your case file. DaRon Stephens, LPC, LCADC, LMHC, MCAP, LMT will inform you verbally on the phone and document the Thirty (30) Minute Phone Service in your chart.
